Multiple Medication Therapy Management Service (MTMS)

The MTMS’ mission is to assist patients whom have multiple chronic medications with the management of their drug therapy in order to improve or maintain their health and prevent or minimize drug therapy-related problems.

By being medication-use specialists and patient care providers, our patients will have improved health status as measured by their clinical, economic, and humanitarian outcomes.

Patient-Related Goals

  • Improve and sustain an acceptable patient compliance to their drug regimen
  • Assist with difficulties in access to medications
  • Incorporate patient participation in self-care and self-management of medications
  • Streamline pharmacy visits and the refill process
  • Assure there is no time lapse between refills
  • Prevent duplication of drug therapy and avoidable drug interactions
  • Identify, monitor for, and prevent or minimize adverse drug events or medication related complications
  • Assist primary doctor in monitoring patients drug therapy and medical conditions, particularly their response to therapy in-between provider visits
  • Provide patient/caregiver education on the purpose of the drug therapy, the particulars in taking their medications, expected side effects and important adverse reactions that need to be reported
  • Improve healthcare utilization and minimize the individual’s healthcare cost
  • Assist in coordination of care with other healthcare providers as it relates to the patient’s drug therapy and overall health
  • Recommend to the patient and primary healthcare provider when necessary preventative measure such as immunizations and routine labs that are established by medical guidelines
  • Improve patient satisfaction with their healthcare as it relates to their drug therapy

Pharmacy Goals

  • Develop an efficient pharmacy model for pharmaceutical care management for patients with multiple medications that allows:
    • One pharmacist to efficiently and knowledgeably follow-up on another pharmacy practitioner's patients
    • Efficient and complete monitoring of patients at patient visits
    • Serve as a teaching site for both students and residents learning the pharmaceutical care process
    • Develop, study and publish innovative systems and models of improving adherence, patient knowledge and overall care

Criteria for Enrollment

A patient may enter the MTMS program by referral from any Medical center healthcare professional that recognizes a patient whom meets any of the following five conditions:

  1. The patient has multiple medications, disease states, or health care providers resulting in diminished coordination of care
  2. Has difficulty in self management of medications
  3. Has difficulty in adherence to chronic medications
  4. Significant lack of understanding or knowledge of chronic drug therapy
  5. The patient agrees to having prescriptions filled at UIC pharmacy
