To make our systems and processes ready, we need your information. Please send the following information to provider@uic.edu

1. Your name & title
2. Your specialty
3. Contact information (address & phone)
4. Your NPI #
5. Your UPIN # (if applicable)

Why should I share this information with the University of Illinois?
In order for us to bill on services that you referred, we need the referring physician NPI #.  Sharing this information is a must and is recommended by the Centers for Medicare and Medicaid Services.  And, as outlined in the Federal Regulation, (The Health Insurance Portability and Accountability Act of 1996 (HIPAA)) you must share your NPI with other providers, health plans, clearinghouses, and any entity that may need it for billing purposes.
